Candida Questionnaire and Score Sheet

I have been impressed by the remarkable uniformity of the histories I have found in my patients with yeast-related problems.

While physical and laboratory examinations provide a physician with some of the clues that leads to a diagnosis, I’ve found that this questionnaire is perhaps my most valuable tool in finding a way to help patients who feel “Sick all over.”

This questionnaire is designed for adults and is not appropriate for children. Since your spouse, companion, friend, or male relative may also be troubled by yeast-related problems, this questionnaire is designed for people of both sexes.

Section A focuses especially on factors in your medical history that promote the growth of Candida albicans.

Section B has a list of 23 symptoms usually present in people with these problems.

Section C consists of 33 other symptoms that are sometimes seen in people with yeast problems – and that are found in people with other disorders. Your total score on this questionnaire should help you and your physician evaluate the role of Candida albicans plays in your problems. While it will not provide an automatic “yes” or “no” answer, it will provide some important clues.
